"Hacked" can have a few different meanings.
1. It is used (incorrectly) as a term for having bypassed the security restrictions of a computer system.
The correct term for such an action is actually "cracked"
Ex. 'YouTube was hacked into on Thursday afternoon.'
Correct term: 'YouTube was cracked into on Thursday afternoon.'
2. It is used as a term for having illegally modified software to avoid the requirement of purchase.
Again, the correct term is usually "cracked."
Ex. 'Paradox made a hack for Photoshop so you can get it for free.'
Correct term: 'Paradox made a crack for Photoshop so you can get it for free.'
3. It is used as a term for having modified software or hardware that you legally
own in order to create additional functionality, achieve better performance,
or simply to learn how it works. Often synonymous with "modder" in hardware
circles, and a freelance programmer in software.
Ex. 'Joe h4xored hiz vid card for max 1337 fps!'
Ex. 'Freddy hacked away at the Linux kernel, trying create a new network card driver'
